We hebben onze eerste werkweek op het Mount Meru Hospital overleefd. Er is zoveel gebeurt deze dagen dat ik niet goed weet waar eerst te beginnen. Ik zal starten met waar ik geeïndigd was. Gedurende deze week stonden we wel met 6 student vroedvrouwen op de Labour Ward en 1 dokter stagair. Het was eigenlijk een beetje te druk om allemaal tegelijk stage te lopen dus hebben we besloten dat ik, Jente en Elise vanaf maandag starten met 3 nachtshiften. Jana en Dana, twee andere studenten van in onze klas gaan volgende week dagdienst doen en zo gaan we elke week afwisselen.
Woensdag morgen kwamen we aan op de Labour Ward en hebben we voor het eerste een briefing meegemaakt, ze duurt welgeteld 5 minuten en we verstaan er niets van. Die dag heeft ook eindelijk iemand van ons een bevalling kunnen doen. Elise was de eerste van ons. We hadden al wat bevallingen kunnen zien en we wisten dat het helemaal niet zo gebeurde zoals wij gewoon waren. De vrouwen in arbeid worden hier niet begeleid door de vroedvrouwen zoals wij gewoon zijn. Er wordt wel elk half uur naar de foetale harttonen geluisterd met een hoorn van Pinard. Wanneer de vrouwen beginnen meepersen wordt er af en toe is gekeken onder het 'verband' om te zien of er nog geen hoofd zichtbaar is en dan pas mag er een 'steriele' set worden geopend en mogen de steriele handschoenen aangedaan worden. De steriele set bestaat uit 2 kochers, 1 schaar, 4 gazen en een bolkommetje met watten in. Deze is dan in een gekleurde stinkende doek gewikkeld en wordt zo in een autoclaaf gestoken om te steriliseren. Ook doen de vroedvrouwen hier 2 steriele handschoenen over elkaar aan, maar van steriliteit is er niet veel sprake. Wanneer ze de handschoenen aan hebben raken ze ondertussen nog allerlei andere dingen aan. Sommige vroedvrouwen proberen wel steriel te blijven, maar vaak lukt dit niet. Ik heb gevraagd waarom ze twee handschoenen over elkaar aandoen en dit was omdat ze met bloed te maken hadden en 1 handschoen was te dun. Ze zijn hier ook heel agressief tegen de vrouwen. Wanneer ze niet goed genoeg meewerken volgens de vroedvrouw wordt de vrouw geslagen. Heel erg om te zien. Soms denk ik echt dat de vrouwen die fel reageren op een vaginaal onderzoek een trauma hebben meegemaakt. Ze huilen heel het onderzoek en ze spannen zich helemaal op. Het was heel erg om te zien. De mannelijke dokters zijn ook heel hard handig. Ze zijn niet erg begaan met de vrouwen, ze zijn er vies van en slaan ook direct.
Op donderdag hebben drie van ons een bevalling kunnen doen. Het was heel druk en we hadden een beurtrol opgemaakt zodat iedereen een bevalling kon doen. Het was eindelijk aan mij. Een vrouw waar ik totaal niet van verwacht had dat ze al ging bevallen. De vrouwen zijn hier zo sterk en stil tijdens de weeën, ik heb echt heel veel respect voor ze. Dus de vrouw waar ik de bevalling van ging doen was een eerste kindje. We moeten ook persee twee steriele handschoenen aandoen dus ik vol stress mijn handschoenen aangedaan en natuurlijk op die moment zitten alle vingers in de verkeerde gaten van de handschoen. Uiteindelijk heb ik alles klaar gekregen en ik had assistentie van een vroedvrouw. Ik had er nog niet mee samengewerkt dus wist niet wat te verwachten. Ze liet me heel de bevalling alleen doen. In tegenstelling tot bij Dana en Jana. Hier kwam de kuisvrouw altijd ertussen om heel het proces te forceren en de baby er gewoon uit te sleuren. Ik heb echt een mooie natuurlijke bevalling kunnen doen. Alles verliep heel vlot en ik was in de wolken dat ik de bevalling zonder hulp volledig zelf op de juiste manier heb kunnen doen. Ik heb een damsteun uitgevoerd, het hoofd traag geboren laten worden, een navelomstrengeling weggehaald, de schouders geboren laten worden en de rest van de baby. De baby heb ik afgenaveld en afgedroogd en afgenaveld en daarna heb ik de placenta laten geboren worden. De vroedvrouw was heel lief en hielp me hier en daar met opruimen en dingen aan te geven. Na de bevalling zei ze tegen mij ‘You did very wel, you are a good midwife’. Aangezien dit mijn 13de bevalling was, was ik heel blij met deze opmerking!
Donderdag avond zijn we voor het eerst naar een social night van projects abroad gegaan. Het was heel leuk en hebben heel wat vrijwilligers leren kennen. We hebben ook pizza kunnen eten en na een week Afrikaans eten smaakte dit echt wel!
Vandaag ben ik voor het eerst mee gaan kijken naar een reanimatie van de baby. Deze verloopt hier ook heel anders dan in België. Ze nemen de baby mee naar een houten tafel. Daar drogen ze de baby nogmaals goed af en proberen te prikkelen zodat de baby goed doorhuild. Ze proberen met een peertje de slijmen uit de mond en neus te halen en daarna geven ze met een aspiratieslang zuurstof in de neus. Ook pakken ze de baby ondersteboven bij de benen vast en slaan ze op de rug en schudden ze met de baby. Verschrikkelijk om te zien! We zien ook in de verloskamers (die niet gebruikt worden) reanimatietafels staan maar ze gebruiken deze gewoon niet. Heel spijtig om te zien.. De baby van vandaag leefde nog toen we de verloskamer verlieten omdat onze shift erop zat, maar hoe het nu verder is gegaan weet ik niet. De baby ademde en de hartslag was goed, maar hij had geen één keer goed doorgeademd en de spiertonus was niet goed. Hopelijk komt hij er goed door.
Dit is in het kort wat we de afgelopen dagen hebben meegemaakt. Ik heb zowat de belangrijkste zaken proberen te vertellen. Het is te veel om alles in detail te vertellen.
